- Mid-sized, busy family practice is seeking a caring, compassionate, responsible nurse to serve as a Nurse Care Coordinator.
- This role includes developing and coordinating care plans, supporting patients and families, and conducting transition-of-care phone calls to ensure smooth discharges from hospitals/rehab.
- Must have excellent communication and organizational skills. Valid Vermont nursing license required.
